ANNUAL WELLNESS VISIT


The Medicare Annual Wellness Visit (AWV) is a yearly appointment with our AWV department and or your primary care provider (PCP) to develop or update your personalized prevention plan. The AWV helps you prevent disease and disability based on your current health and risk factors.

During your visit we will cover the following:

  • General Health 
  • Tobacco and Alcohol Use 
  • Nutrition 
  • Physical Activity 
  • Sleep 
  • Functional Status Assessment 
  • Pain Assessment 
  • Home/Safety 
  • Functional status evaluation 
  • Fall risk screening 
  • Cognition screening 
  • Depression screening (PHQ-2, PHQ-9) 
  • Behavioral risks 
  • Activities of Daily Living 
  • Establish list of current providers and suppliers 
  • Update your medical/social and family history 
  • Physical Exam includes your height, weight, body mass index, blood pressure, visual acuity, and other factors based on your assessment 


Establish a written screening schedule for the beneficiary including any age or gender appropriate screenings such as:

  • Colorectal cancer screening 
  • Mammography
  • Annual pap screening 
  • Bone density testing 
  • Ultrasound screening for Abdominal Aortic Aneurysm (AAA) 
  • Prostate cancer screening 
  • Hepatitis C and HIV 
  • Influenza, Pneumococcal, Hepatitis B vaccinations
