To schedule your first appointment, please call our office at (541) 747-4300. During your phone conversation with our office staff, you will be asked to provide a valid phone number. This number will be used to send you a link with your patient registration information. You’ll need this link to access and print the new patient packet, which should be completed before your visit.

Please arrive 10 minutes early to your first appointment and bring a valid photo ID and your current insurance card.

If you are unable to print and complete your new patient paperwork at home, please arrive 20 minutes early so we can provide a printed copy for you to fill out in the office.
