Chelsea Loy, LCSW


Chelsea is a native New Englander, and earned her Master of Social Work degree at the University of New Hampshire. Because of her passion for whole-person, patient-centered healthcare, she specialized in primary care behavioral health. In particular, she developed clinical interest in providing behavioral interventions to assist with managing chronic health conditions. 

This expertise is valuable in her work as a Clinical Social Work Associate at Northwest Medical Homes where Chelsea collaborates with patients, families, and the healthcare team to create a plan to improve overall well-being. In her practice, she utilizes evidence-based approaches including, Focused Acceptance Commitment Therapy, to support patients in engaging in lives that are meaningful to them. 

Chelsea enjoys expanding her collection of random fun facts. Fun Fact: If she wasn't a therapist, she would be a fruit/vegetable farmer. She appreciates witnessing resilience and growth in lives of people and plants!
